EMPLOYMENT TERMS AND CONDITIONS
You must agree to the terms below when submitting an application for any position:
It is unlawful in Massachusetts to require or administer a lie detector test as a condition of employment or continued employment. An employer who violates this law shall be subject to criminal penalties and civil liability.
I hereby authorize Sudbury Water District and its agents to conduct whatever inquiry they deem necessary (including but not limited to my education and employment experience and any other aspects of my background relevant to my proposed employment) including all statements made by me in my application for employment. I also agree to release to Sudbury Water District and its agents as well as any person to whom such inquiry is directed, from any liability arising directly or indirectly from any such investigation.
I certify that, to the best of my knowledge, the statements made in this application are true, complete, and accurate and further agree that such statements may be investigated at any time and if found to be false or misleading will constitute sufficient reason for refusal of employment or termination of employment from Sudbury Water District. I further agree to abide by all company rules and policies. I understand that my employment is for no definite or fixed period of time and that neither hours of work, which may be assigned to me at any time, or any other act or circumstances shall constitute a guarantee of fixed terms and conditions of employment.
